About this service

Maintaining mobility and independence at home is one of the most important factors in long-term wellbeing. A fall can be life-changing, but with the right support, many falls are preventable. Open Heart Care takes a proactive approach — working to reduce risks in the home environment while encouraging as much independent movement as possible.

Our carers are trained in safe manual handling and the correct use of mobility aids and equipment. We take time to understand each person's ability and confidence, providing the right level of assistance — never too much, never too little — to help maintain strength, balance and function.

We work collaboratively with occupational therapists, physiotherapists and community nursing teams, ensuring that our approach to mobility support is consistent with any wider rehabilitation goals.
