About this service

For many people, remaining in their own home during the final stages of life is deeply important. Open Heart Care provides compassionate end-of-life care that prioritises comfort, dignity and quality of life — supporting not only the individual, but their family through what is often the most profound and difficult of times.

Our nurse-led team works in close partnership with GPs, district nurses, hospice teams and palliative care specialists to ensure that care is coordinated, responsive and consistent with the individual's wishes. We support advance care planning conversations and are experienced in communicating sensitively with families about what to expect.

We are here around the clock, providing reassurance and practical support whenever it is needed. Our goal is simple: to ensure that every person in our care feels safe, comfortable and loved in the place they call home.
